The resident or the legally responsible party is requested to fill out an application for residency. A copy of guardianship, power of attorney, trustee appointment, living will, and/or health care directives must be provided to the Home at time of admission.
Arrangements for admission and assumption of responsibility are to be made by the family and/or the physician. Either the resident or the legally responsible person must sign all admission authorizations. Residents are admitted to this facility only on the written order of the attending or admitting physician. The choice of physician lies with the resident or the legal responsible party. A resident's right to our services is not contingent upon contributions in any form to ensure admission or continued stay.
Residents of all creeds and races are welcome and no discrimination shall be practiced in the utilization of beds. A physical examination shall be done at the time of admission and at least annually thereafter.
We are licensed to admit residents whose care is skilled (Medicare), also called Level I, intermediate (long-term care chronic) Level II, and those covered by the Medicaid and Medicare programs in both categories. A review of the Resident's Bill of Rights has to be made by the resident and/or the legally responsible person and a statement signed that the rights are understood. A resident is responsible for his/her own affairs unless a full guardianship is in effect and shall be consulted accordingly. A payment in advance for 30 days care is required at time of admission. Medications brought into the nursing home must be approved by our registered pharmacist before Nursing Home staff can administer. It is the nursing home's preference that medications are purchased through the consulting pharmacy; however, the choice is yours.
